Improving Follow-up After a Behavioral Health-Related Emergency Department Visit
We are connecting patients with appropriate and timely follow-up care following a visit to the emergency department (ED) for behavioral health concerns.
Our Focus
About one in five children in the United States suffers from a mental health condition—but more than half of children who suffer from depression never receive treatment. Evidence shows that timely follow-up within 30 days of an ED visit for a behavioral health concern improves outcomes and reduces the likelihood of a return visit to the ED.
Our Impact
Our focus is on any 12-17 year old with a behavioral health ED visit. Further, PHIN explicitly focuses on eliminating socioeconomic and racial equity gaps to ensure that ALL patients have improved outcomes.
Continuous Improvement
Teams are currently evaluating interventions at successful sites to determine which initiatives PHIN will test beginning January 2024.
