Addressing Social Determinants of Health (SDOH)
Our Focus
Social determinants of health (SDOH) are environmental conditions that affect health outcomes and risks. Examples include pollution exposure, transportation access, and literacy skills. SDOH are grouped into five domains:

Neighborhood and Built Environment
Social and Community Context
Our Impact
SDOH affect anyone and everyone. Evidence demonstrates that minorities and lower-income household units have increased social risks and social needs and may also face additional hurdles to address these barriers.
PHIN develops and shares evidence-based, whole child models of care, child health metrics and benchmarks, education programs, advocacy materials, and population health research to improve equitable child health outcomes.
Continuous Improvement
SDOH is at once a universal and hyper-local issue. PHIN teams review data, current best practices, and local contexts to develop a collaborative approach to test and implement lasting changes.
