Our Network
Mission
Our network aim is to develop and share evidence-based, whole child models of care, child health metrics and benchmarks, education programs, advocacy materials and population health research to equitably improve child health outcomes.
Vision
We are bringing health systems together to positively transform the lives of the nation’s children.
Background
The Pediatric Population Health Improvement Network (PHIN) was born in 2021 from a desire among participants in the Accountable Health Learning Collaborative (AHLC) to coordinate quality improvement efforts to enhance population health outcomes.
Since 2016, the Children’s Hospital Association (CHA) has facilitated the AHLC, a venue for children’s hospitals to share and learn as they evolve the systems, relationships and capabilities to assume increasing accountability for populations of children.
The AHLC facilitates the exchange of best practices, accelerates the effectiveness of institutional strategy and progress in value-based care (VBC) arrangements and enables exploration of new approaches among participating organizations. With support from the AHLC, participating teams embarked on the design and development of PHIN.
Since launch, we have grown to six partners and are actively working on improving behavioral health outcomes while simultaneously developing a project to address Social Determinants of Health.
